What is your concern?
Briefly summarize your concern. Please be as specific as possible and provide the
following information:
It is GPEB’s policy and practice to keep the identity and personal information of anyone who reports suspected wrongdoing confidential;
however, GPEB may be required by law to reveal the identity and contact information of any person who
reports suspected wrongdoing and any information the person provides to GPEB.
Please note that if you do not wish to be identified, you should not include any information in the report
that could identify you as it is possible that information you provide may still expose your identity,
despite lack of name and contact information, if someone is able to determine who you are from the details you provide in the report.

under sections 26(a), (b) and (c) of the
Freedom of Information and Protection of Privacy Act
(FoIPPA)
and under sections 27(3) and 86(2) of the Gaming Control Act for the purposes of reviewing your concerns
and, if necessary, to conduct an investigation.

programs and activities relevant and necessary to GPEB’s functions and authorities.
